Our Care Model

Bartram Lakes empowers our Residents with dignity and independence while also ensuring they receive the care they need. We are proud to have an Extended Congregate Care (ECC) license as well as a standard ALF license. An ECC license means that our Residents are able to age in place and allows us to have other therapeutic and nursing services available in-house.

On-Site Medical Care

An important component of your loved one being able to age in place, is ensuring they receive routine medical care such as teeth cleanings, dermatology check-ups, hearing aid assistance, yearly physicals, etc. Your loved one is welcome to choose a service provider, but below is a list of preferred medical providers that we know and trust for our Residents and Elders.
