Life at Bartram Campus

Our Bartram Campus is located in southern Duval County, right off of I-95 and Old St. Augustine Road. We are conveniently located near Baptist South Hospital, as well as many shopping, dining and entertainment options. Our Bartram Campus is fortunate to have many types of rehabilitation services available, including Brooks Rehabilitation Inpatient Hospital – Bartram Campus.

Bartram Lakes Assisted Living

Bartram Lakes is a 61-unit assisted living facility that features permanent and respite-care stays for Jacksonville seniors. Our Residents enjoy walking trails, a fishing pond, beautiful living spaces, a solarium with wall-to-wall windows and a wide array of social activities.

Bartram Crossing Skilled Nursing Facility

Bartram Crossing is our skilled nursing facility connected to Bartram Lakes. Offering 74 private rooms and 13 semi-private rooms, Bartram Lakes can accommodate 100 Guests. Bartram Crossing is divided into three “neighborhoods,” each offering its own dining and recreation areas. Bartram Crossing also has a therapy courtyard, healing gardens and a 4,500 square foot therapy gym equipped with the latest evidence-based technology to allow for specialized, multi-disciplinary therapy approaches. Our highly trained Brooks therapists use specialized equipment, such as a LiteGait, automobile simulators and more to deliver therapy services to our Bartram Crossing Guests. Bartram Lakes Residents and The Green House® Elders also have access to therapy services at Bartram Crossing.

The Green House® Residences Memory Care

The Green House® Residences consist of two 12-room memory care homes for seniors living with Alzheimer’s or other forms of dementia. Each Elder at The Green House® Residences enjoys the comforts of home while receiving specialized care from our team of Shabaz, which are caregivers specific to The Green House® model of care.
