Brooks Rehabilitation has proudly served our community for more than 50 years and opened our assisted living and memory care locations in 2015 to address the needs of our senior population Jacksonville.

Amenities
Bartram Lakes offers our Residents a wide-range of amenities including 24/7 nursing available, a fitness center, hair salon, nail salon, barbershop, 115-acre campus with walking trails, billiards, activities center, computer stations and so much more.

Dining
Our Residents are able to enjoy meals whenever their appetite strikes! Our executive chef has items available all day in the main dining room, as well as lite eats in our Café Bistro.

Activities & Events
Our dedicated Activities Director schedules activities, outings, social hours and events on a regular basis. Cooking demos, crafts, shopping excursions, beach outings and movie theater visits are all regulars on our monthly activities calendar.

Living Spaces
Bartram Lakes features 61 beautiful studio, one-bedroom and two-bedroom apartments. All rooms come unfurnished, so Residents are encouraged to bring items from home to add their own personal flare to their residence.

Compassionate Care Staff
Bartram Lakes has nursing available 24/7, including Certified Nursing Assistants and Medical Technicians to ensure our Residents receive needed care at any time.
How is Bartram Lakes different from other assisted living facilities?
Bartram Lakes operates under an Extended Congregate Care (ECC) license as well as a standard ALF license. An ECC license means that our Residents are able to age in place without having to transfer to different facilities. An ECC license also allows us to have other therapeutic and nursing services available in-house so residents can truly have a full system of care available for them.
As of 2020, there are 87 assisted living communities in Duval County. Out of those, only nine communities, specific to assisted living, have an ECC licensure. The others have only a standard license or a standard license with a Limited Nursing Services (LNS).
Brooks System of Care
Bartram Lakes Residents have access to the entire Brooks Rehabilitation system of care, which has been trusted in the Jacksonville community for more than 50 years. From inpatient, outpatient, home health, skilled nursing, assisted living and memory care to adaptive sports, pediatrics and research, the Brooks system is powerful, innovative and evidence-based.
Dedicated Aging Services Campus
The Bartram Campus features assisted living, memory care, skilled nursing and an inpatient rehabilitation coming in 2022.
Expertise in Therapy
Our expertise in rehabilitation is trusted as being one of the most advanced in the region. Our integrated treatment teams include certified therapists from many disciplines who receive ongoing training in the latest approaches in therapy.
